We’ve noticed that there is an issue with the Smartsheets form system that is affecting the autofill function in some fields. I have been in touch with Smartsheets support on this and have received the guidance below. If you find that you are having issues with Smartsheets automatically changing info on your form, please try the steps below.

I understand you have some users reporting that when they submit Forms, some fields Autofill with incorrect values. I am happy to advise!
Other users have reported this behavior and it appears to happen based on the settings of the browser being used to fill out the form. In this case, our assistance is limited.
For the users reporting this issue, we recommend the following options below:
- Changing the Autofill settings to Ask instead of Automatic: This will ask them to either accept or the Autofill.
- Clearing the cache & cookies, then restart the browser before submitting the form
- Using another browser or using the private/incognito window when submitting the form
- For some browsers, they can determine which sites/forms they would like to autofill and those they don't want to.
Here are the instructions for the suggestions below using the Chrome browser:
To change Autofill settings in Chrome:
- Click the three dots in the upper right corner of Chrome (…)
- Click Settings, then “Addresses and more”
- Turn off “save and fill addresses”
To clear Cache and Cookies in Chrome:
- Click the three dots in the upper right corner of Chrome (…)
- Click History, then click History on the menu that comes up
- Click “Clear browsing data” on the left side
To open an Incognito window in Chrome:
- Click the three dots in the upper right corner of Chrome (…)
- Click “New incognito window”
